Heart of Illinois Continuum of Care

 

 

The Vision of the Heart of Illinois Homeless Continuum of Care (“CoC”) is to end homelessness in Fulton, Peoria, Tazewell and Woodford Counties (“CoC service area”) by ensuring access to permanent, safe, decent and affordable housing for persons and families facing homelessness.

 

The mission of the CoC, a united coalition of community systems, is to assist persons and families facing homelessness to obtain permanent housing, self-sufficiency and an enhanced quality of life through comprehensive, coordinated services.

 

The purpose of the CoC is to minimize the trauma and dislocation of homelessness to individuals, families, and communities by assisting in the planning, development and coordination of housing services; obtaining and directing funding; providing community education; and advocating for clients. To achieve the above mentioned purpose, the CoC will seek to:

  • Rapidly re-house homeless individuals and families as appropriate;

  • Maintain a coordinated response among service providers to ensure continuity of services;

  • Assess needs and identify gaps in services for persons facing homelessness in the CoC service area on an ongoing basis;

  • Support planning, funding and development of services to meet prioritized needs within the CoC service area;

  • Plan, develop and sustain options to meet the housing needs of people facing homelessness; and

  • Promote access to and effective utilization of mainstream programs.

 

The responsibilities of the CoC are operating the CoC, designing and operating an HMIS, CoC planning, and reviewing/approving funding applications.
